SEVENTH FIRE OUTDOOR EXPERIENTIAL LEARNING OVERNIGHT TRIP CONSENT FORM

Dear Parent(s) and Guardian(s),

 ELEMENTS OF RISK

Overnight camping trips provide excellent opportunities for student learning.  As in anything, risks may be involved or present.  Such risks could result from, but are not limited to, forms of transportation (school bus, hiking, weather etc.).  Accidents related to such activities may occur and cause injury to a student or students through no fault of Lac des Mille Lacs First Nation (LDMLFN), the LDML Education Centre, Seventh Fire Secondary School, Seventh Fire Junior High,  its employees and volunteers, the transporter, or the facility at which an activity takes place.

I AM AWARE THAT:

  • Trip attendance numbers are limited for both safety and to provide an effective learning environment. Sign up will be conducted on a first come first serve basis (permission forms and fees must be submitted to be added to the list). Please pay camp fee no later than seven days prior to departure – at which point fees become non-refundable (fees include prepaid expenses for permits, supplies, groceries and additional staffing). Extenuating circumstances can be discussed on a case by case basis. We appreciate your understanding in this matter.

  • Students will be required to hike, canoe, sleep in tents, cook, and engage in physical activity;

  • Students are responsible for having appropriate clothing & footwear (which will be reviewed before departure); 

  • Students are responsible for carrying their own water bottle, food, supplies, personal items (notebooks, etc.);

  • Students acknowledge that they will surrender their devices to the Land-Based team for the duration of the overnights. Devices will be locked in a secure box inside the emergency vehicle and returned upon completion of the trip.  Any devices that are not surrendered on trip are the students responsibility and damages may occur (weather, water, etc.). The school will not be held liable for any such loss or damage;

  • Drugs and alcohol, including marijuana substances, are not permitted on the trip. A student who decides to bring any of these substances will be removed from the trip. The parent/guardian who is listed on this waiver will be immediately notified and will be required to pick their child up from the camping location; this includes all e-cigarettes and vaping devices. 

  • If a student becomes ill on the trip, the school and parent/guardian will be notified.  The school will endeavor to bring the student back to school where the parent/guardian will be responsible for pick up; 

  • If a student is injured, they will be treated by a qualified Wilderness First Responder, and will be taken to the hospital for medical attention if deemed appropriate. The school and parent/guardian will be notified and kept up to date when it is safe and feasible to do so.

  • The school does not provide any additional accidental death, disability, dismemberment, or medical expense(s) insurance for students;

  • The student agrees to follow all direction given by the Instructor/Teacher while on the camping trip, with special emphasis on adhering to all safety directives; 

  • The student agrees to participate in the activities set out in a responsible manner that is conducive to successfully acquiring set skills and knowledge; 

  • Although not a school requirement, this trip will meet curriculum expectations of each students’ class work;

  • Students who are unable to attend will be provided with alternate paperwork assignments/activities to be complete during the days of the overnight trip;

  • Only students that have attended and passed the swim proficiency test may attend a canoe trip;

  • The required fee can be paid via School Cash Online.
